Course Description:

This evidence-informed two-hour course examines the anatomy, biomechanics, and pathophysiology of acute facet-mediated wry neck, with emphasis on synovial inflammation, meniscoid entrapment, and intra-articular pathology. Participants will develop a practical differential diagnosis, recognize serious competing disorders, and learn patient-centered management strategies, including appropriate application of high-velocity, low-amplitude (HVLA) spinal manipulation, informed consent, and evidence-based clinical decision-making.

Hourly Breakdown:

  • Hour 1 – Anatomy, Pathophysiology, and Clinical Assessment
    • Cervical facet joint anatomy, biomechanics, synovial tissues, meniscoids, and mechanisms contributing to acute torticollis.
    • Pathophysiology of synovitis, plica/meniscoid entrapment, intra-articular loose bodies, and segmental hypomobility.
    • Clinical examination, differential diagnosis, imaging considerations, and recognition of serious competing disorders requiring referral.
  • Hour 2 – Evidence-Based Clinical Management
    • Conservative management options and appropriate patient selection for manual therapy.
    • Safe application of HVLA spinal manipulation, informed consent, patient communication, and technical considerations.
    • Post-treatment expectations, home care, follow-up, documentation, risk management, and clinical outcomes.

Learning Objectives

  • Describe the anatomy, biomechanics, and pathophysiologic mechanisms underlying acute torticollis, including synovitis, meniscoid entrapment, and facet joint dysfunction.
  • Differentiate acute facet-mediated torticollis from competing musculoskeletal, neurologic, vascular, and infectious disorders that require alternative management or urgent referral.
  • Develop evidence-informed treatment plans that incorporate appropriate clinical assessment, patient education, informed consent, and conservative management strategies.
